Community Mental health services

Stellar Health Services provides mental health treatment in the form of individual, family, and/or group therapy. Our team consists of licensed clinical social workers, and mental health counselors specializing in depression, anxiety, grief and loss, stress management, life changes, and life skills, to name a few. 

Targeted case management

Medicaid Mental Health Targeted Case Management services provide case management to adults with a serious mental illness and children with a serious emotional disturbance to assist them in gaining access to needed medical, social, educational, mental health / behavioral health services, and other services. This service is one of the minimum covered services for all Managed Medical Assistance (MMA) plans, and Dual Special Needs Plans (DSNP) serving Medicaid enrollees. Some sample services that targeted case managers link / facilitate / coordinate for Medicaid enrollees that are eligible / qualify: medical services / specialists, long term care, Home Health Care (HHA), Medicaid / medical transportation, ADA transportation, WIC / SSI / TANF / EBT (food stamps), Disability Low Income Home Energy Assistance Program (LIHEAP), low-income housing application, Section-8 housing application, educational services (I.E.P., Charter, etc.), mental health services (therapy, group therapy, ABA, PSR, etc.), hospital discharge, medication management, and natural / social / community resources.

psychosocial rehabilitation (PSR)

PSR is a skill based service that trains individuals on skills necessary for independent living.  PSR focuses on developing and improving several aspects of well-being including: Social (Relationships, family, boundaries, communication), Vocational/Educational (Coping, skills, motivation, technology, job search), Financial (Budgeting), Psychiatric (symptom management), Health / Medical(Maintain consistency of care, medication compliance), Basic Living Skills(hygiene, meals, safety, planning, chores), Housing(maintenance of safe environment).
